Dear Carlos,

    One of the mechanisms that has been more studies is related with TLR
expression. In such case some TLR may trigger a more intense
pro-inflammatory response and others a more regulatory and the same
pathogen express molecules recognized by different TLR. Additionally the
same TLR as TLR-2 may trigger TNF and IL-10 production. Another factor is
the subpopulation of Monocytes that is activated.
    Anyway the mechanims are not completely understood.

Indeed, IL-10 is always present whenever inflammatory cytokines are there.
But I wonder what are the mechanisms behind this phenomenon. One thought
would be that the same stimulus that trigger one icytokine also trigger
IL-10 like in one molecule-one receptor-one cytoplasmatic/nuclear pathway
model. But this is certainly too simplistic since there are lots of
Leishmania molecules and many receptors and the PRR that lead to IL-10
apparently are not the same that trigger icytokines.

Yes, I understand that nature has provided the immune response with many
negative feedback loops in order to prevent exaggerated immune response
like it happens in kala-azar and sepsis.

    It is common the high production of pro-inflamatory cytokines and
anti-inflammatory cytokines as IL-10. In the majorirty of the cases IL-10
is produced in an attempt to down modulate the inflammatory response. In
the case of visceral leishmaniasis production of IL-10 is induced by
parasite antigens as well pro-inflammatory cytokines.

I am trying to understand why in human VL the anti-inflammatory IL-10 is
systemically strongly expressed together with IL-6, INF-g, and other
pro-inflammatory cytokines.

I was looking at the mechanisms behind it but found little hints. Please
let me know if you have any response for this.
